How to Make a Medicine List Pharmacists Can Use in Two Minutes
To make a medicine list, write down everything you swallow, spread, spray or inhale: prescriptions, over-the-counter products, vitamins, herbs and creams. For each, add the dose, the time, the reason and who prescribed it. Keep one copy on paper and one on your phone. Show it before adding any new capsule.

What goes on a medicine list?
Everything that goes in or on your body on a regular basis, and the things you take only now and then. If you would not think to mention it, write it down anyway.
Think of the kitchen drawer, the bathroom shelf and the bag you carry. Pharmacists often find the surprise in the bag: a daily antacid, a sleep aid, an herbal tea capsule. These matter because many interactions come from the small, forgotten items.
- Prescription pills, patches, inhalers, drops and injections.
- Over-the-counter pain, allergy, cold and stomach products.
- Vitamins, minerals, protein powders and herbal products.
- Creams, ointments and lip products you use often.
What does a good medicine list template look like?
One row per product, with six columns. The table below is a template you can copy into a notes app or print.
| Name | Dose | When | Why | Who said so | Started |
|---|---|---|---|---|---|
| Example: Reviralis Complete capsules | 2 capsules, per the label | Daily, 20-30 min before a meal | Your own reason | Self | Date |
| Prescription (name) | mg and form | Times a day | Condition | Prescriber | Date |
| Cream or ointment | Amount | Where, how often | Reason | Self or clinician | Date |
Add allergies and past reactions at the top. Add the pharmacy name and phone number at the bottom. Date the sheet so a clinician knows how fresh it is.
How do you list a supplement with many ingredients?
Write the product name and photograph the Supplement Facts panel. A multi-ingredient label tells the pharmacist more than the product name does.
Reviralis Complete has 40 names on its label: 22 vitamin and mineral lines with printed amounts and an 18-name proprietary blend of 661 mg with no single amounts. A photo of the panel saves you from copying all of it by hand. Pharmacists check the printed lines, such as B6 at 12 mg or zinc at 10 mg, against your other products and your prescriptions.
For blends, say plainly that the amounts are not printed. The order of the names is useful: the label lists them from most to least by weight (21 CFR 101.36). Our page on the Supplement Facts panel reads it line by line.
Several names on the label, including green tea leaf extract, licorice root extract and bilberry fruit extract, are known to interact with some medicines. A pharmacist can only check what is on your list.

See packsHow does a pharmacist use your medicine list?
They scan for duplicates, interactions and timing problems, then tell you what to ask your prescriber. Bring the list to a pharmacy counter before you start something new.
Some products compete for absorption, such as minerals and certain antibiotics (NIH ODS, Calcium). Others add up, such as two products with the same vitamin. The label warns about prescription medicine, especially antidepressants, blood thinners and medicine affecting the nervous or cardiovascular system, so mark those lines first.
Keep taking every prescribed medicine, including any antiviral, while you sort this out. Do not change a dose because of a list. Our medicine check page and the post on questions to ask a pharmacist give you wording for the counter.
How do you keep a medicine list up to date?
Update it the day anything changes, and read it aloud at each checkup. A list that is six months old is worse than none.
- Pick a fixed day, such as the first of the month, to review it.
- Cross out stopped products with the date, rather than deleting them.
- Share a copy with the person who would speak for you in an emergency.
- Take the list, or a phone photo of it, to every appointment.
The label says to consult your healthcare provider before use if you take prescription medicine. No source tests Reviralis itself, so the list is how you and your pharmacist fill in what the label cannot.
FAQ01
Questions about medicine lists
Do vitamins and herbs belong on a medicine list?
Yes. Vitamins, minerals and herbal products can affect how other medicines work, so list them with the same care as prescriptions. Include the brand, the dose per serving and how many you take each day.
Should I include creams and lip balms?
Yes, if you use them often or on a large area. Skin products can be absorbed or cause reactions, and a clinician may want to know. A short note with the product name and how often you apply it is enough.
How often should I update my medicine list?
Update it the same day anything starts, stops or changes, and review it monthly. Read it again at every checkup. An outdated list can mislead a pharmacist, so dating each version helps.
Can a phone note replace a paper list?
A phone note works if you can open it quickly in an emergency. Keep a paper copy in your wallet or bag too, since phones run out of power. Make sure someone you trust knows where it is.
