What should a symptom diary template include?
A symptom diary template needs seven columns: date, symptom, where, how bad from 1 to 10, what you took, what you did that day, and sleep. Write it the same time each day. Include any Reviralis capsules or other supplements, with the dose. A diary helps a clinician decide; it does not diagnose anything.

Why keep a symptom diary at all?
A diary turns a vague memory like "it was a bad week" into dates and facts a clinician can use.
Picture the exam room. The clinician asks when it started, how long it lasted and what you took. Most of us cannot recall that three weeks later. A page with dates answers all three in seconds.
Mayo lists fever, stress, fatigue, sun and hormonal changes as common cold sore triggers (Mayo Clinic, Cold Sore Symptoms and Causes). A diary is the plain way to notice whether any of those line up with your own days. It shows patterns. It does not prove a cause.
See a clinician for a diagnosis, because other conditions look like herpes. A diary supports that visit and does not replace it.
What columns belong in a symptom diary template?
Use seven columns and keep each entry to one line.
| Column | Example entry |
|---|---|
| Date and time | Oct 12, 8 am |
| Symptom | Tingling, burning, headache, tiredness |
| Where | Lip, left side |
| How bad (1 to 10) | 4 |
| Medicines and supplements taken | Name, dose, time |
| What happened that day | Late night, sun, a fever, a stressful meeting |
| Sleep (hours) | 5.5 |
Copy those headings into a notes app or a paper notebook. Both work. Paper is easier to hand over.
How do you log medicines and supplements?
Write the product name, the dose and the time every time you take it, including capsules, vitamins and anything for pain or sleep.
For Reviralis Complete, the label directs two capsules daily with an 8 oz. glass of water, 20 to 30 minutes before a meal. The serving is 2 capsules and a bottle holds 30 servings. If you take it, record the days you take it and the days you skip. Do not guess later.
List prescription antivirals too. Keep taking any prescribed antiviral exactly as directed, and never stop or swap one because of what a diary seems to show. A diary cannot tell you whether a supplement works, and no source we found shows that Reviralis or any of its 40 names changes outbreaks. Our medicine list post shows how to keep the master list.
- Write "none" on days you take nothing.
- Note changes the same day, not at the end of the week.
- Bring the bottle labels to the visit, or photograph them.

See packsWhat mistakes make a diary useless?
The two big ones are filling it out from memory and judging it too early.
Writing it late
Entries made days later drift from what really happened.
Too many columns
Seven is plenty. A longer form gets abandoned.
Blaming one thing
One bad night plus one flare is a coincidence, not a finding.
Review the diary after four weeks, not four days. Look for repeats. Circle anything that shows up three or more times and bring that to the visit.
What should you bring to the appointment?
Bring the diary, a full medicine and supplement list, and three questions written down.

FAQ01
Questions about keeping a symptom diary
How long should I keep a symptom diary?
Four weeks is a good first run, since it can show repeats. Keep going until you see the clinician. If symptoms get worse or are severe, do not wait for the four weeks to end. Call the office.
Is an app better than a paper diary?
Either works if you write daily. An app can sort entries and set reminders. Paper is easy to hand to a clinician and has no battery. Choose the one you will actually fill out.
Should I record every supplement I take?
Yes. Write the product name, dose and time, even for vitamins. A clinician can only check interactions with what is on the list. Include Reviralis days and skipped days if you use it.
Can a diary show what triggers a cold sore?
It can show patterns, such as sun or a poor night of sleep before a flare. It cannot prove cause. Mayo lists fever, stress, fatigue, sun and hormonal changes as triggers. A clinician helps read the pattern.
What if my diary shows nothing?
That is useful too. A clean month tells a clinician something real. Keep the diary for another month if symptoms continue, and mention any changes in sleep, stress, medicine or supplements.
